KindQuest Supports LLC is a Medicaid-enrolled, OLTL-approved Home and Community-Based Services (HCBS) provider aligned with Pennsylvania's Community HealthChoices (CHC) program. We also provide community-based support and employment-focused services in partnership with workforce and vocational programs, helping individuals build independence, skills, and meaningful community participation. We support safe hospital discharges, rapid service activation, and person-centered care for adults across Central Pennsylvania.
Getting Started with Community HealthChoices (CHC)
Pennsylvania's Community HealthChoices (CHC) program provides long-term services and supports to eligible adults and seniors.
The CHC Enrollment Process
- Enrollment through the IEB: Enrollment into CHC begins through the Pennsylvania Department of Human Services Independent Enrollment Broker (IEB). The IEB helps individuals understand eligibility, enroll in the program, and select a CHC health plan.
- Service Coordination: Once enrolled, the CHC health plan assigns a Service Coordinator who works with the individual to authorize services and select an appropriate provider.
- Service Delivery: KindQuest Supports LLC delivers non-medical, home-based services once authorization is approved, supporting individuals to live safely and independently in their homes and communities.
